We have been awarded the highest – Level 3 — recognition under the NCQA Physician Practice Connections — Patient Centered Medical Home program. What does that mean for you? As a patient-centered medical home, you and your provider are the core of a team managing your care. So whether you need medical, dental or behavioral health care, our health centers are committed to providing coordinated primary care when you need it most.
And with our Telehealth program you will have access to specialty care from world-class hospitals and academic centers without ever leaving our center. Providers and patients can have face-to-face consultations with an off-site specialist in areas such as dermatology, dentistry, behavioral health, nutrition, pulmonology, ENT, and diabetic retinotherapy…to name a few.
