Our Care Management program assists children and adults with mental health and/or substance use disorders.

We know how difficult it is to navigate the complex healthcare system. That’s why our care management programs are designed to assist you in getting the services you deserve.

If you live in these counties*, you may qualify for these services:

  • Assistance with medical, mental health, substance use, and social services appointments
  • Linking you to resources for affordable housing, food, clothing, and transportation
  • Ensuring that all of your providers are working together
  • Supporting your health and wellness

Medicaid Funded Programs

Medicaid-based care management programs like Health Homes of Upstate NY (HHUNY) for adults or Children’s Health Home of Upstate NY (CHHUNY) programs, will connect you with a care manager who will work with you to not only get the medical services you need but also to help you be successful in other parts of your life.

Who qualifies for Adult Care Management Services (HHUNY)?

FLCH adult patients with Medicaid are eligible for these services if they also have:

 

2 chronic health conditions

(e.g. asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder)

Serious Mental Illness (Bipolar Disorder, Schizophrenia, etc.)
HIV/AIDS

AND

Has significant behavioral, medical, or social risk conditions

FLCH patients who are ages 5 to 21 years old and have Medicaid are eligible for CHHUNY services if they also have:

Who qualifies for Care Management for Seniors?

Frustrated with the healthcare system? Do you find it difficult to get your medications when you need them? FLCH patients who are over 60 years of age (who are not farmworkers) and have Medicare, or are not insured, or pay for their services themselves, may qualify for this program.

Care Management Services includes:

  • Interpretation
  • Transportation assistance
  • Linkage to:
    • Dental services
    • Medical services
    • Behavioral health services
    • Reproductive health services
  • Assistance With:
    • Health Insurance
  • Caseloads can/will consist of:
    • Barriers to care, reduction in hospitalization, ED, and Urgent Care visits
    • Reduction in hospital readmission within 30 days
    • Patients’ adherence to the treatment plan
    • Increase medication adherence (take meds)
    • Fall Risk Screening assessments
    • Referrals to specialty care

The grant-funded Geriatric Care Management program’s primary purpose is to provide a level of complex care coordination to FLCH patients 60 years of age and older (who are not farmworkers). The Geriatric Care Manager will be the center point of communication with you to ensure all your providers are working together. That includes providing the best care, identifying risk factors, and referring/navigating systems and barriers to your care. We will also assist in reducing high medical costs such as prescriptions, medical services, and treatment at the Emergency Department if it’s not needed.

Care Management for Farmworkers?

Who qualifies? FLCH patients who are farmworkers employed in agriculture or hired to work for agricultural production, including harvesting of crops, caring for livestock, working in dairy farms, and greenhouses​.

Services includes:

  • Reproductive health
  • Education for immunizations and hygiene precautions
  • Accessing and navigating health services
  • Safe lifting
  • Diabetes
  • High blood pressure
  • Pesticide safety and awareness
  • Heat and sun exposure safety
  • Nutrition
  • Tobacco
  • Healthy living environments
  • Access to community support services

Mobile Medical visits to farms:

  • On-site COVID and influenza vaccines
  • COVID testing
  • Health screening services include a health assessment, mental health assessment, education, height/weight, blood pressure check, blood glucose check, and routine HIV screening
  • Additional immunization through external medical providers and pharmacies
  • Chronic Care visits which include follow-up visits for those who can be treated/managed at their home, by a medical provider

  • Health history including reproductive health, tobacco use, mental health assessment, Blood Pressure check, blood glucose check, HIV tests, occupational assessments, social determinants of health assessment
  • If there are areas of concern, referrals will be made to a medical provider in the health center for further evaluation: case management will be provided for the patient as well.
Q: How much are Dental screenings during Incamp? What is covered during the screening?

A: $0/INCAMP Dental Screenings

  • We start by asking patients if any teeth are bothering them, then do a soft tissue evaluation, and visual check to determine if any areas of concern are present. That may include visual decay, broken teeth, retained root tips, swelling, and abscesses. An appointment for cleaning is scheduled at which time X-rays will be taken to complete the exam. Any restorative work or extractions are scheduled as needed.
