Sodus Community Health is located at 6341 Ridge Road and offers medical, dental, and behavioral health services.

We provide personal, individualized care with an emphasis on wellness and prevention. Our providers believe that listening is the best medicine. We get to know your goals and concerns so we can deliver the best care for you.

Dr. Rachel Long is board certified in Family Medicine caring for all ages, including children. Our dentists in Sodus include our Chief Dental Officer, Anthony Mendicino and Christopher Lehfeldt. 

At Sodus Community Health, we were recognized by the National Committee on Quality Assurance (NCQA) for providing comprehensive primary care for children, youth, and adults. We were awarded the highest— Level 3—recognition under the NCQA Physician Practice Connections—Patient-Centered Medical Home program.
