Finger Lakes Community Health (FLCH) has the Moderna and Janssen (Johnson & Johnson) COVID-19 vaccines for all medical patients who are 18 years of age and older. We also offer the Pfizer COVID-19 vaccine for all medical patients who are 5 – 17 years of age.
Moderna
Moderna Initial Vaccination Series
The Moderna COVID-19 vaccines is a two-dose vaccination series. The second dose is scheduled 28 days after your first dose.
Moderna Additional Dose
An additional dose (this is not a booster shot) is recommended for people who have a qualifying medical condition associated with immunosuppression. Click here to see what conditions qualify for an additional dose.
The additional dose must be given at least 28 days after your second dose. Studies showed inadequate immune responses to only two doses in these populations, so this is considered part of the initial dose series and is not the same as a booster shot.
If you have a qualifying medical condition associated with immunosuppression and have already completed your two-dose series, please complete the COVID-19 Vaccination Request Form below.
If you have not completed your two-dose series, your additional dose will be scheduled at the completion of your second vaccination.

Note: Patients receiving the additional dose will need to sign a document stating that they are immunocompromised and are eligible.
Moderna Booster
A booster shot is meant to maintain an already adequate immune response in people who previously completed the initial vaccination series.
Vaccine booster shots can be given to individuals 18 years and older, six months after your second or additional dose.
Janssen (Johnson & Johnson)
Janssen (Johnson & Johnson) Initial Vaccination Series
The Janssen (Johnson & Johnson) vaccine is a one-dose vaccination.
Janssen (Johnson & Johnson) Booster
A booster shot is meant to maintain an already adequate immune response in people who previously completed the initial vaccination.
Booster shots can be given to individuals 18 years and older, two months after your primary vaccination.
PFIZER (5 – 11 years of age)
At this time, Pfizer is the only COVID-19 vaccine approved for children 5 – 11 years of age.
Pfizer (5 – 11) Initial Vaccination Series
The Pfizer COVID-19 vaccine for individuals ages 5 -11 is given in a two-dose vaccination series. The second dose is scheduled 28 days after the first dose.
At this time there is not an additional dose or a booster for the 5 – 11 age group.
Pfizer (12 – 17 years of age)
Pfizer (12 – 17) Initial Vaccination Series
The Pfizer COVID-19 vaccine for individuals ages 12 -17 is given in a two-dose vaccination series. The second dose is scheduled 21 days after the first dose.
Pfizer (12 – 17) Additional Dose
An additional dose (this is not a booster shot) is recommended for individuals ages 12 -17 who have a qualifying medical condition associated with moderate to severe immunosuppression. Click here to see what conditions qualify for an additional dose.
The additional dose must be given at least 28 days after the second dose.
If the individual has a qualifying medical condition associated with immunosuppression and has already completed the two-dose series, please complete the COVID-19 Vaccination Request Form below.
If the individual has not completed the two-dose series, the additional dose will be scheduled at the completion of the second vaccination.

If you are an FLCH medical patient and you meet the age requirement, you can request a vaccination by clicking here and completing the COVID-19 Vaccination Request Form. A member of our team will reach out to you to schedule your vaccination within three (3) business days.
There will be no out-of-pocket cost to you for this vaccine*.
*If you are insured, a small administrative charge will be billed to your insurance company.
